Idiopatic Neonatal Gastric Perforation

Abstract
This is the second case report of spontaneous neonatal gastric perforation in Korea, which was xperienced in a 2-day-old Korean male full-term infant. The patient was born by a middle-aged woman who was suspected of having hydramnios during the pregnancy. He was operated on about 75 hours after the onset and died about 312 hours after the surgery.
The reasons for delayed surgery were that no free air was demonstrated in the X-ray examinations of abdomen and that several times of stool passage during hospitalization obscured the diagnosis.
A review of literatures is also made on the etiology, diagnosis, treatment and the prognosis of the subject. The hydramnios may have contributed as an etiological factor in this case, and the absence of subphrenic free air may be explicable by the location of perforation in this case. The perforation was located on the greater curvature of stomach just behind the gastric attachment of gastrocolic omentum and the free air may have been confined in the lesser sac.
